In order to compete as a Rodriguez athlete, you must complete the paperwork on www.athleticclearance.com. New athletes must create a profile and complete the online paperwork. This must be done every school year. Once completed you will gain access to the confirmation page. Please print out the confirmation page, signed and dated by both a parent and athlete, and return to coach.

Please take this document to your doctor to complete, during your physical appointment. This document needs to be filled out by your doctor, signed, and dated. Sports physicals are only valid for one school year!
